Accidental Medical Expenses Claim The original medical, hospital bill(s) and diagnosis issued by the medical practitioner
Permanent Total Disablement Claim The original medical report showing the extent of injury and the percentage of permanent disability
Death Death certificate issued by the appropriate authorities and Visum et repertum
